At Second Chance Society, Inc., we understand that many individuals face barriers to stability due to financial constraints. Providing funding for vital documents such as a Birth Certificate or State ID Card is often the catalyst for meeting a clients basic needs and taking the next step toward a more secure future.

Case Manager has identified, printed, and completed appropriate birth certificate application with client, to include information to whom check is payable and total amount requested.

Case Manager has ensured that reason for request is employment or vocational training related.

Get Assistance with an ID or Birth Certificate
To receive assistance with the funding of a Birth Certificate or a Florida ID Card, a Case Manager must complete the “BC/ID Program Application”. Once the application is successfully submitted, a member of our staff will be in touch within 3-5 business days to review and discuss request. If awarded, checks can be picked up from our office or, if necessary, will be directly mailed to the referring Case Manager within 7-10 business days.
